Every office leader and/or building manager is required to carry out office work health and safety to realize a healthy, safe, and comfortable office and healthy employees, safe, fit, performing and productive. Minister of Health as the leader in The Ministry of Health Office, assigned the Secretariat General's Personnel Bureau to prepare coordination and administration materials for conducting annual medical checkups (MCUs) for employees, especially Ministry of Health officials given their strategic role in the organization. But in its implementation, the participation of officials as MCUs participants has not been maximized due to the absence of quota information and scheduling procedures that are still manual by telephone. Therefore, through the design of the MCUs information system for Ministry of Health officials are expected to maximize the participation of them in implementing the MCU. The purpose of this study is to develop model of MCUs scheduling and personal health record. This research will design an MCUs information system based on android mobile using prototyping method. An analysis of this research is carried out by observations, document reviews, interviews with the parties involved and literature studies aimed at producing an effective and efficient system. Information presented on the web includes the MCU implementation report while in android in addition to scheduling the MCUs results are also displayed and recommendations for follow-up based on the MCUs results.
